Parental Consent and Involvement Form for Coaching Sessions

Coaching Program Overview:

This coaching program is designed to support and enhance your child's development in personal growth. Sessions will be conducted per booking and will last approximately 30 minutes to 1 hour depending on booking preferences.

Consent:

I, the undersigned, hereby give my consent for my child, to participate in the coaching sessions provided by HappyMindski. I understand that the sessions will focus on personal growth.

Confidentiality:

I acknowledge that the information shared during the sessions will remain confidential between the coach and my child, with exceptions only in cases where disclosure is required by law or when there is a risk of harm.

Parental Involvement:

I agree to be involved in the coaching process by:

  1. Attending scheduled meetings with the coach to discuss my child's progress.

  2. Providing feedback and support to my child as recommended by the coach.

  3. Communicating any concerns or questions to the coach promptly.

Liability Waiver:

I understand that while the coaching sessions are designed to support my child's growth, HappyMindski is not liable for any outcomes related to the coaching program.

Success depends on the minor's effort, motivation, and commitment, and the coach cannot be held responsible for a lack of progress. 

This emphasizes that the minor is ultimately responsible for their choices and behavior during and after coaching sessions.
